Rickettsia parkeri
R. parkeriは、ロッキー山紅斑熱（RMSF）の原因菌であるR. rickettsiiと近縁種である。 R. parkeriリケッチア症とRMSFは、発熱、頭痛、発疹などの症状が似ていますが、一般的にマダニ付着部位に接種痕（右図）を認めます。
Where Found
R. parkeri リケッチア症は、南東部と中部大西洋岸、およびアリゾナ州南部の一部で湾岸ダニによって媒介されます。
Incubation Period
2–10 days
Signs and Symptoms
R. parkeri rickettsiosis is characteristically less severe than RMSF and almost always associated with an inoculation eschar (ulcerated, necrotic lesion) at the site of tick attachment. Several days after an eschar appears, the following can develop:
- Fever
- Headache
- Rash (sparse maculopapular or papulovesicular eruptions on the trunk and extremities)
- Muscle aches
NOTE: R. parkeri rickettsiosis can be difficult to distinguish from RMSF and other spotted fevers, especially during early stages of these diseases. Eschars are uncommonly identified in persons with RMSF.
General Laboratory Findings
- Mildly elevated hepatic transaminases
- Mild leukopenia
- Mild thrombocytopenia,
診断の確定は検査に基づくが、臨床症状を示唆する患者において抗菌薬治療を遅らせるべきではない。
実験室診断
- 綿棒、全血、皮膚生検でのPCRによるリケッチアDNAの検出
- ペア血清サンプルの間接免疫蛍光抗体（IFA）法によるIgG特異抗体価の4倍の変化（通常は上昇）を実証する。
注意：R. parkeri の種レベルの検査は市販されていません。 RMSF抗体検査は市販されており、しばしば交差反応を示します。
注意：IgM抗体はIgG抗体よりも特異度が低く、偽陽性を生じやすいとされています。
注意：急性抗体検査の結果は、単独で確定診断に頼ることはできません。
